The Story Behind SynerDose™ & Why I Spent Four Years Rethinking Skincare

When I set out to create KaramMD Skin, I wanted to accomplish something that sounds relatively simple but turned out to be incredibly challenging...

I wanted to develop a comprehensive anti-aging skincare system that delivers meaningful results without requiring people to follow complicated routines or tolerate unnecessary irritation.

I believed the answer wasn’t necessarily finding stronger ingredients or using more products. It was finding a better way to combine the ingredients we already know can improve the appearance and health of our skin.

That belief ultimately led to the development of SynerDose™, the proprietary formulation technology behind KaramMD Skin.

SynerDose represents a different approach to skincare formulation.

Rather than relying on high concentrations of a few individual hero ingredients, we combine multiple complementary targeted ingredients at carefully selected concentrations, with the goal of optimizing the performance of the entire formula while maintaining the tolerability necessary for consistent daily use.

It’s an approach that took more than four years of experimentation, patient feedback, and formulation adjustments to bring to life. And it grew out of something I’ve always believed about achieving results, whether we’re talking about skincare, nutrition, exercise, or just about anything else we do to improve our health.

The best results rarely come from one individual action. They come from combining the right things in the right way, and doing them consistently enough to make a meaningful difference.

That’s the thinking behind SynerDose.

What Is SynerDose™?

SynerDose™ is KaramMD Skin’s proprietary formulation technology designed to optimize skincare efficacy by combining multiple complementary targeted ingredients at carefully selected concentrations, while minimizing irritation that can interfere with consistent daily use.

SynerDose™ technology is built around three interconnected principles:

  • Efficacy: Combining complementary ingredients that address multiple signs and mechanisms of skin aging, rather than relying on a single hero ingredient to do most of the work.
  • Tolerability: Carefully balancing concentrations and ingredient combinations to reduce unnecessary irritation and help maintain a healthy skin barrier.
  • Consistency: Creating comprehensive formulations that people can comfortably incorporate into a simple daily routine and continue using over time.

The important distinction is that SynerDose isn’t about making skincare gentler at the expense of results. It’s about optimizing efficacy while making consistent use possible.

And that last part is more important than many people realize.

You can have the most scientifically advanced skincare routine in the world, but if it’s too complicated, irritating, or difficult to maintain, you’re unlikely to experience its full potential.

I wanted to change that.

Why I Believed There Had to Be a Better Way

As a facial plastic surgeon specializing in facial rejuvenation, I’ve always considered skin quality an essential part of helping people look as young as they feel.

For years, I was recommending fairly complicated skincare routines to my patients. And I had a very specific reason for doing so.

Skin aging isn’t caused by one process. It’s the result of multiple biological changes happening simultaneously: declining collagen and elastin, slower cell turnover, weakening of the skin barrier, inflammation and oxidative stress, moisture loss, and changes in pigmentation.

I wanted my patients to address all of these things, not just one or two.

But with the products available at the time, the only way I knew to accomplish that was to combine multiple products, each designed around a particular ingredient or benefit. So I would put together routines involving eight or more steps, carefully selecting serums, creams, and targeted ingredients to make sure we were covering as many aspects of skin aging as possible.

From a scientific standpoint, the approach made sense to me. But in practice, I began recognizing a major problem.

Very few patients could maintain these routines consistently.

Some found them too time-consuming or expensive. Others struggled with irritation from layering multiple targeted ingredients, particularly when those ingredients were formulated at concentrations that weren’t necessarily intended to be used together. In some cases, the combinations contributed to skin barrier disruption, making the skin more sensitive and reactive.

And when patients couldn’t tolerate the routine or simply became overwhelmed by it, they would start skipping steps, discontinuing products, or abandoning the routine altogether.

That was a frustrating realization because my intention had always been to give patients the most comprehensive skincare possible. Yet the very complexity I believed was necessary was becoming an obstacle to achieving the results we wanted.

I began to recognize that I needed to rethink the entire approach.

What if we could maintain the comprehensiveness of an eight-step routine without requiring patients to actually perform eight steps?

That question changed the way I thought about skincare formulation.

The Realization That Changed Everything

There was another important realization that fundamentally changed the way I thought about skincare.

For years, I had been carefully assembling routines for my patients using individual products that contained scientifically studied ingredients. I understood what retinol could do, what vitamin C could do, and the benefits of other targeted ingredients. I was selecting products based on the best available evidence and combining them to address the different aspects of skin aging.

But there was something missing.

While the individual ingredients had been studied, the complete routine I was putting together had never been tested as a whole.

And that was a problem.

I could make educated decisions about which products to combine, but I couldn’t know exactly how those products would perform together. Their concentrations hadn’t necessarily been selected with the other products in mind. Their combined effects on tolerability weren’t fully understood. And although each product might have evidence supporting its benefits, that didn’t tell me what the finished routine would accomplish.

I began to recognize that there was an important difference between putting together a collection of effective products and developing an effective skincare system.

I wanted to control the process from beginning to end.

Rather than choosing from products that had already been formulated independently, I wanted to start with the objectives: address the multiple mechanisms of skin aging, select complementary ingredients supported by science, and then work with cosmetic chemists to develop formulations in which the concentrations, compatibility, stability, and tolerability were all considered together.

But I also wanted to go one step further. I wanted to test the finished routine, not just rely on studies of the individual ingredients.

That meant developing the Trifecta as a coordinated skincare system and then evaluating the complete routine through independent clinical testing, using the products together as they were intended to be used.

This was the fundamental shift behind SynerDose™.

It wasn’t simply about combining more ingredients or making skincare easier. It was about taking responsibility for the performance of the entire system, from the original formulation objectives through to the finished routine.

For the first time, I could move beyond what I believed a combination of ingredients should accomplish and actually measure what the finished system was doing.

That was the approach I wanted to pursue, even though I knew developing it would be far more complicated than assembling products that already existed.

Four Years of Trial, Error, and Adjustment

When I began developing what would eventually become the Trifecta, there wasn’t an existing formula or blueprint for the specific system I wanted to create.

We were starting from scratch.

I worked closely with trusted cosmetic chemists and a laboratory, bringing them my objectives, my wish list of ingredients and benefits, and the clinical perspective I had developed over years of treating patients.

My role wasn’t to be the chemist. It was to establish the North Star for what we were trying to accomplish and evaluate whether each formulation was getting us closer.

I wanted a comprehensive system that could address the multiple signs of skin aging, deliver meaningful results, and remain well tolerated enough for people to use consistently.

That sounds relatively straightforward, but it turned out to be incredibly challenging.

The greatest difficulty was finding a way to incorporate such a robust combination of targeted ingredients without overwhelming the skin.

Every ingredient had a purpose, but ingredients don’t exist in isolation once they’re combined into a formula. Their concentrations, compatibility, stability, and effects on the skin all have to be considered together. Adjusting one component can influence the performance or tolerability of others.

And this is where the process became much more involved than I originally anticipated.

We would develop a formulation, test it with small groups of patients, gather feedback, and then return to the laboratory to make adjustments. Sometimes that meant reducing the concentration of a particular ingredient. Other times it meant changing the combination, adjusting the formulation, or reconsidering how ingredients were being delivered.

Then we would test again.

This process repeated itself over and over, with ingredients added, concentrations refined, and formulas repeatedly adjusted. We weren’t simply combining existing products or following a predetermined recipe. We were developing an original system around a very specific objective.

And throughout those four years, I kept coming back to the same question: Could we make this more effective without making it harder for people to tolerate and use?

It took more than four years of experimentation, testing, and refinement before we arrived at the formulas that became the Trifecta.

Looking back, there’s something interesting about how much complexity went into creating something intended to make skincare so much simpler.

But that was the whole point. I wanted the complexity to be in the formulation, not in the daily routine.

The First Moment I Knew We Were Onto Something

One of the most encouraging moments during development wasn’t necessarily seeing a dramatic improvement in someone’s skin.

It was seeing patients with extremely sensitive skin tolerate our formulations.

These were people who had historically struggled with products containing targeted anti-aging ingredients. They would try retinol, vitamin C, or other active products, only to experience irritation that prevented them from continuing.

Yet with the formulations we were developing, some of these same patients were finally able to use a comprehensive combination of targeted ingredients comfortably.

For me, that was a major breakthrough.

We were beginning to demonstrate something I had believed was possible: that comprehensive skincare didn’t have to mean aggressive skincare, and that a thoughtfully balanced combination of ingredients could be more practical than relying on high concentrations of individual actives.

Of course, these were early observations from small groups of patients, not controlled clinical trials. But they gave us the confidence to keep going.

And once we had developed a system that patients could tolerate, the next question became even more important.

Did it actually work?

From an Idea to Measurable Results

As a physician, I’ve always been careful about distinguishing what we believe we’re seeing from what we can objectively demonstrate.

We were receiving encouraging feedback from patients and early customers. People were telling us their skin looked better, felt healthier, and that they were finally able to maintain a consistent routine.

But testimonials and personal observations, no matter how encouraging, can be influenced by expectations and observation bias.

I wanted independent evidence.

That’s why we had the finished Trifecta system evaluated through third-party clinical testing, rather than relying solely on published research about the individual ingredients.

That distinction matters because a collection of scientifically studied ingredients doesn’t automatically mean the finished product will deliver the same results. The formulation itself needs to perform.

In our four-week clinical testing:

  • 94% of participants showed improvement in the appearance of lines and wrinkles
  • 100% showed improvement in firmness
  • 97% showed improvement in moisture.
  • We also measured improvements in skin tone and texture.

At eight weeks, the results continued to be encouraging:

  • 100% of participants showing improvement in lines and wrinkles and moisture
  •  91% showing improvement in firmness.

For me, this was an important milestone.

We had spent years developing a system based on the idea that carefully combining complementary ingredients could deliver comprehensive results while maintaining tolerability. Now we had independent finished-product measurements supporting its efficacy across multiple signs of skin aging.

The study didn’t isolate the contribution of every ingredient or prove that each combination was synergistic. But it did provide objective evidence that the finished system was working, which was ultimately what mattered most to me.

It gave us confidence that the approach we had spent so many years developing could deliver meaningful improvements in the appearance of aging skin.

Five Years Later

More than five years after introducing the Trifecta, one of the most rewarding parts of this journey has been seeing how KaramMD Skin has become part of people’s everyday lives.

Today, we have tens of thousands of subscribers and an incredibly loyal community of customers who have made these products part of their daily routines.

To me, that means something.

The independent clinical studies gave us objective evidence that the finished system could deliver meaningful improvements in the appearance of aging skin. But seeing people continue to use the products over the years has been equally rewarding because consistency was such an important part of what I set out to accomplish.

I’ve always believed that when people see results from something they’re doing, it reinforces their motivation to continue. They recognize that the time, effort, and money they’re investing are worthwhile.

And that creates a positive cycle: consistency supports results, and results encourage consistency.

That’s what I hoped to achieve when we began developing SynerDose™ more than a decade ago, and it’s what continues to guide how we formulate our products today.

Looking back, I’m proud of the four years of development, the clinical testing, and the technology we created. But what matters most to me is knowing that we’ve helped make comprehensive skincare something people can realistically incorporate into their lives.

Because the ultimate measure of a skincare system isn’t how impressive its ingredient list looks on paper. It’s how well it performs for the people who use it.

And that’s what SynerDose™ was designed to accomplish.
