She Already Looked Great at 60. So Why Did She Choose a Facelift?

She had followed my work for several years and realized that facial rejuvenation didn’t necessarily mean looking like a different person. It could mean looking like herself again.

And that’s really what this story is about.

Something I’ve noticed throughout my career is that most of us carry around a mental image of ourselves that doesn’t necessarily change as we get older.

For many people, that image is somewhere in their 30s. It’s when our features have matured, but the more significant changes associated with facial aging haven’t yet become apparent.

I always say that youthful facial appearance depends on two essential things: healthy skin + youthful facial shape.

It’s important to understand the difference between what skincare can accomplish and what requires surgery.

• You can have beautiful, well-cared for skin but still develop sagging cheeks, jowls, and changes in the jawline as the deeper facial tissues descend. 
• Conversely, you can have a beautifully lifted face following surgery, but if the skin itself has significant sun damage, wrinkles, and discoloration, the overall appearance will still reflect those changes.

In Jamie’s case, her skin was already exceptionally well preserved. What needed attention was the shape of her face and some of the volume changes that had occurred over time.

When I evaluate a face for rejuvenation, one of the first things I look at is its overall shape, particularly from the front.

A youthful face typically has a more heart-shaped appearance, with fullness in the upper and middle face and a narrower, more defined jawline. As we age, the tissues of the cheeks, jawline, and neck gradually descend, creating heaviness in the lower face and a more square or rectangular appearance.

The objective isn’t simply to tighten the skin. It’s to restore the natural anatomy without creating the pulled or stretched appearance that people understandably associate with traditional facelift surgery.

The second component was facial volume.

As we age, we experience changes in facial fat and bone that contribute to hollowing, particularly around the eyes and temples. These changes can make the face look tired even when the skin itself is healthy.

Jamie had some volume loss around her upper and lower eyelids, temples, and portions of the lower face.

I addressed these areas with carefully placed fat transfer, restoring the fullness that had diminished over time.

Interestingly, Jamie also didn’t need upper or lower eyelid surgery. Her eyelid skin was remarkably well preserved, and there wasn’t significant excess skin that required removal.

This is an important principle in facial rejuvenation. Just because someone is having a facelift doesn’t mean they need every possible procedure. The goal is to identify what has actually changed and correct only what needs correcting.

Of everything that stands out about Jamie’s case, this may be the most important lesson for anyone reading this article:

But healing doesn’t stop there. Residual swelling gradually improves over the following months, and the results continue to become more refined.

By four months, Jamie had reached a point where we could really appreciate the beautiful changes in her face.

When you compare Jamie’s before-and-after photographs, the changes are remarkable.

She doesn’t look pulled, stretched, or overfilled. She looks like Jamie.

And because her skin was already so healthy, the restored facial contours are complemented by the beautiful luminosity that we associate with youthful skin.

I often talk about the way light reflects off healthy skin. People call it a glow, but what we’re really appreciating is the smoothness, hydration, and uniformity of the skin surface.

When you combine that with youthful facial shape, the overall effect can be remarkable.

But what matters most to me isn’t whether someone thinks Jamie looks 35, 40, or any other particular age.

And the best approach isn’t necessarily to do more. It’s to understand exactly what has changed and address those changes in the most appropriate way.

Can someone really look 20 years younger after a facelift?
A well-planned facelift can create a significant improvement in facial appearance, particularly when tissue descent is a major contributor to aging. However, results vary depending on anatomy, skin quality, and the procedures performed. The goal should be natural rejuvenation rather than achieving a specific numerical age.

Why can’t skincare eliminate jowls and sagging cheeks?
Skincare primarily improves the quality and appearance of the skin. Jowls and sagging cheeks result largely from changes in deeper facial tissues that topical products cannot reposition. This is why skincare and surgery address different aspects of aging.

Does everyone need fat transfer with a facelift?
No. Fat transfer is appropriate when volume loss contributes to an aged appearance. The amount and placement should be individualized. In some areas, lifting the descended tissues restores enough fullness that additional volume isn’t necessary.

How long does recovery from Vertical Restore take?
Most patients experience the greatest swelling and bruising during the first one to two weeks. Many feel comfortable returning to social activities around two weeks, although recovery varies. Residual swelling can take several months to resolve, and the final result continues to refine over time.

Does good skin quality make facelift results better?
Healthy, well-preserved skin can significantly complement surgical rejuvenation. A facelift restores facial shape, while skin quality contributes to the smoothness, luminosity, and overall youthful appearance of the face. Both are important.

What can I do now to help my skin age better?
