As a plastic surgeon specializing in facial rejuvenation, I’ve spent years advising patients on the best in-office treatments.

There’s a lot of misinformation out there—promises of miracle fixes, ineffective treatments, and some trends I wouldn’t recommend.

Today I want to cut through the noise and give you my trusted guide to in-office skin treatments that actually work. These treatments are scientifically-backed, widely used, and here to stay because they genuinely improve skin quality.

While no treatment, no matter how advanced, can replace a daily skincare routine formulated with key active ingredients, in-office treatments can be a beneficial supplement to your consistent, foundational skincare habits.

Let’s break down the treatments that I personally recommend for maintaining youthful, healthy skin.

My Favorite In-Office Facial Rejuvenation Treatments

01.

Microneedling: The Power of Collagen Stimulation

Best for: Fine lines and wrinkles, skin texture, collagen production

Microneedling is one of my go-to recommendations for skin rejuvenation. This treatment uses tiny needles to create controlled micro-injuries, prompting the skin to produce collagen and elastin—two essential proteins that keep skin firm and youthful.

Over time, regular microneedling treatments can significantly improve the integrity of the skin, making it more resilient against the visible signs of aging. It is particularly effective for reducing the appearance of acne scars and mild hyperpigmentation by promoting accelerated skin renewal. The gradual thickening of the dermis also leads to firmer, more lifted skin, reinforcing its long-term anti-aging benefits.

Key Benefits of Microneedling:

  • Stimulates natural collagen production
  • Improves skin texture and firmness
  • Reduces fine lines and wrinkles over time
  • Can be done in a series for optimal results (every 4-6 weeks)
  • Low risk, high reward
  • Can also be done on the neck

Advanced Options:

  • Microneedling with PRP (Platelet-Rich Plasma): Uses growth factors from your own blood to enhance healing and collagen production. The PRP gets absorbed in the tiny little holes created from the treatment, and as a result, the growth factors in the plasma stimulate collagen production more efficiently.
  • Microneedling with Radiofrequency (RF): Adds thermal energy to boost collagen stimulation by creating a larger area of injury. But, RF should be used cautiously to avoid causing some damage to the facial fat, which is precious, and you definitely don't want to lose it because that’s what keeps the skin looking supple.

02.

Chemical Peels: Targeting Dull Skin and Pigmentation

Best for: Uneven skin tone, dark spots, dullness, fine lines and deeper wrinkles

Chemical peels work by peeling away the top layer of dull, dead skin cells, encouraging the regeneration of a newer, healthier layer of skin post-treatment. During this process, fine lines and wrinkles are softened, skin texture feels smoother, and any pigment that happens to be in the upper dermis is removed.

Light chemical peels, such as glycolic, salicylic, or lactic acid peels, are excellent for maintaining a bright, even-toned complexion without significant downtime.

This controlled exfoliation process stimulates collagen production, helps with deeper wrinkles or scarring, boosts skin elasticity, improves pigmentation issues, and further reinforces the skin’s natural barrier and moisture retention for a more hydrated, youthful appearance.

Key Benefits of Chemical Peels:

  • Improves skin texture and radiance
  • Fades pigmentation and sun spots
  • Helps reduce fine lines, deeper wrinkles, and acne scars
  • Encourages a smoother, more even complexion

03.

Laser Skin Resurfacing: Transformative Skin Renewal

Best for: Sun damage and pigmentation, deep wrinkles, texture irregularities

Lasers are among the most powerful tools for skin rejuvenation, but there are many types of lasers that vary by intensity, wavelength, and target area. Choosing the right laser depends on the specific skin concern you are trying to address.

To better understand their applications and benefits, laser facial treatments can be broadly categorized into two main types: non-invasive and invasive.

Non-invasive laser treatments use light to target damaged, pigmented skin cells. They break up the pigmentation using the wavelengths and usually require multiple treatments to see results.

Alongside these laser treatments, it’s important to use pigment-reducing skincare ingredients like Retinol, Vitamin C, botanical lighteners, and Niacinamide to help suppress the melanin and prevent the pigment from coming back post-treatment.

The invasive lasers create a thermal injury to resurface the skin and these types of laser treatments are much more intense and require a longer recovery. They deeply resurface, ultimately promoting collagen production and stimulating deep layers of tissue remodeling, resulting in a more rejuvenated appearance.

These types of lasers can treat stubborn wrinkles like around the mouth, they can reduce the appearance of age spots, refine pore size, and restore the skin’s overall smoothness and luminosity.

Key Benefits of Non-Invasive Lasers (IPL, BBL):

  • Intense Pulsed Light (IPL) and Broadband Light (BBL) target pigmentation and redness.
  • Ideal for sun damage, brown spots, and broken capillaries.
  • Requires multiple sessions (at least 3-6) for best results.

Key Benefits of Invasive Lasers (CO2, Erbium YAG):

  • Fractionated CO2 and Erbium YAG lasers use thermal injury to resurface the skin.
  • Used for more severe signs of aging, including deep wrinkles and significant pigmentation.
  • Longer downtime but dramatic improvements in skin texture and tone.

04.

Botox: The Gold Standard for Preventing Expression Lines

Best for: Fine lines and wrinkles like forehead lines or crow’s feet

Botox and other neuromodulators (Dysport, Xeomin) temporarily relax facial muscles to prevent the deepening of expression lines.

As the skin ages, it naturally thins due to the loss of collagen. With repeated facial expressions, these weakened areas begin to form static wrinkles that remain even when the face is at rest.

Botox helps preserve a smoother skin surface by reducing repeated muscle contractions that lead to those static wrinkles. Botox not only softens existing lines but also slows down the formation of deeper, more permanent wrinkles.

The best strategy for Botox is to use a skincare routine that also promotes natural collagen production to maintain elasticity and firmness. This will improve the skin's integrity and allow it to remain more flexible so that the need for Botox becomes less.

Key Benefits of Botox:

  • Softens dynamic wrinkles caused by facial expressions
  • Prevents fine lines from becoming deep-set wrinkles
  • Quick, virtually painless treatment with no downtime
  • Effects last 3-4 months on average

05.

Dermal Fillers: Enhancing Facial Volume Naturally

Best for: Loss of volume, under-eye hollows, temporal region, lip enhancement

Dermal fillers, typically made from Hyaluronic acid, plump up the area of injection to restore lost volume. I like filler as a potential tool for specific things: fixing a small line or crease, lip enhancement, addressing hollowness in temples, augmentation of the chin, and a small amount under the eyes can make a big difference.

There's a lot of value is using filler for slight modifications and minimal volume loss that is typically seen in a younger person. However, once you get older, there is significantly more volume loss which leads to skin laxity and at that point fillers are not going to do the job without distorting your appearance. With that, I always emphasize that fillers need to be used conservatively.

Key Benefits of Fillers:

  • Restores youthful volume where needed
  • Softens subtle lines and folds
  • Can help augment certain areas, like lips or chin
  • Fills hollows around eyes and temples for a more refreshed look
  • Results are immediate and can last 6 months to 2 years
  • Hyaluronic acid fillers can be dissolved

06.

Professional Facials: Skin Maintenance and Deep Cleansing

Best for: Regular skin upkeep, hydration, gentle exfoliation

Professional facials can help deep clean pores, exfoliate dead skin, and certain treatments, like Hydrafacials, can even infuse hydration back into the skin. There are many different types of professional facials which can be specifically tailored to your skin's needs and goals.

While not as corrective as lasers or microneedling, facials play a role in maintaining a clean, clear complexion, and work well to support at-home efforts, like Polish.

Key Benefits of Regular Facials:

  • Enhances skin hydration and glow
  • Removes impurities and unclogs pores
  • Can be customized to your skin's needs
  • Lowest risk treatment with little, to no downtime

The Ultimate Truth: Skincare is Your Foundation

Every one of these treatments can enhance and improve skin quality, but none of them replace a solid daily skincare routine. It's best to think of in-office procedures as a way to supplement the hard work you’re already putting in with your daily skincare routine.

Your skin’s foundation relies on two crucial habits:
  1. Sun Protection: UV exposure accounts for up to 80% of visible aging. Daily SPF is non-negotiable if you want youthful, healthy skin
  2. Active Ingredients: Ingredients like Retinol, Vitamin C, Niacinamide, Peptides, and antioxidants keep skin firm, bright, and resilient. This is why I created the KaramMD Trifecta—to simplify the most effective skincare essentials into a streamlined, easy-to-follow system.

The Optimal Skin Care Strategy

True skin rejuvenation comes from a balanced approach—one that starts with daily skincare and is supplemented by targeted, professional treatments to support long-term skin health and vitality.

A consistent, life-long skincare with key active ingredients alone can protect against premature aging, stimulate collagen production, and help you to maintain an even, radiant complexion.

Once you have a skincare routine in place, professional treatments can enhance and refine your results, addressing specific concerns such as deeper wrinkles, stubborn pigmentation, or pronounced loss of elasticity.

Think of these treatments as tools that fine-tune and accelerate the benefits of your daily regimen, helping you reach your skin goals more effectively.

By taking a strategic approach—prioritizing skincare first and incorporating treatments as needed—you create a long-term, sustainable plan for maintaining beautiful, youthful skin.

Use this guide to navigate your options and confidently choose the treatments that best complement your skincare routine, ensuring lasting results and a complexion that reflects the care you invest in it every day.
